Medical Insurance

Full-time employees are eligible for medical insurance on the first day of the month following the employee’s 
90 day probationary period. Information and enrollment forms will be mailed from the HR Department. 
To assist you with the cost of this insurance, First Med pays a portion of each eligible employee’s health 
insurance. Employees are responsible for paying the balance through payroll deduction.
 
401K
First Med provides eligible employees with a 401K Qualified Retirement Plan, which is an excellent means of 
long term savings for your retirement. Full-time employees who have completed one (1) year of service are 
eligible for this program.
First Med will match up to 4% of your gross wages per pay period.
 
Aflac
Aflac is an elective insurance offered through First Med.
Full-time employees are eligible for enrollment after the 90-day probation period.
 
Guardian
First Med offers Guardian for employee life insurance. All full-time employees will receive 10,000.00 of life 
insurance from First Med for the term of employment. We
also offer elective insurance through Guardian including dental and additional life insurance.
